Shift: 8:00 AM - 4:30 PM

Experience:

18 months of Home Health experience is required.

3 months of experience with Meditech is preferred.

Requirements:

Candidates must have a license prior to placement start.

Candidates must live at least 50 miles away from this facility in order to be considered a traveler when applying for this role.

COVID vaccination required for onboarding. Religious and medical declinations accepted.

Flu vaccination required for onboarding. Declinations not accepted.

Weekend Requirements: Every 4-6 weekends/Holiday Coverage Required if needed.

THIS IS A HOME HEALTH NEED! The physical therapist is responsible for providing physical therapy services to clients of the agency. The primary function is the provision of direct patient care, including collaboration with other team members and participation in the development of the patient's comprehensive plan of care. Rehabilitation equipment including but not limited to: splints, orthoses prostheses, wheelchairs, ADL equipment Modalities. Candidate required to be Home Health Certified .

1.5+ acute nursing care (hospital base) and Home Care experience.

Acute care experience preferred.

Certified home health experience including proficiency with OASIS required and NON-NEGOTIABLE.

Maximum days off: 5 days

1 reference from last 24 months (Manager/Supervisor reference type)

Driver's license required
